Course Title: Community Advanced Pharmacy Practice Experience

Course Description: Applies principles of pathophysiology, therapeutics, and communication to the pharmacy-care management of individual patients in a community setting. In collaboration with other members of the health-care team, and under the supervision of a clinical preceptor, offers identification of appropriate drug therapy and monitoring requirements for common pathophysiologic processes, and, when indicated, modification of population-based treatment strategies based on the unique characteristics of individual patients.
